چکیده
M e t h o d s Data sources: 5 databases, bibliographies of relevant studies, and contacting authors. Study selection and assessment: Randomized controlled trials (RCTs) ≥ 21 days in duration comparing 1 of the 5 gastroprotective strategies with nonselective NSAIDs alone on outcomes of GI toxicity. Quality assessment included randomization procedures, allocation concealment, blinding, and follow-up. Outcomes: Primary outcomes included serious GI complications (e.g., hemorrhage, hemorrhagic erosions, recurrent upper GI bleeding, perforation, and pyloric obstruction) and symptomatic ulcers.

The effectiveness of five strategies for the prevention of gastrointestinal toxicity induced by non-steroidal anti-inflammatory drugs: systematic review.
OBJECTIVES To assess the effectiveness of five gastroprotective strategies for people taking non-steroidal anti-inflammatory drugs (NSAIDs)--H2 receptor antagonists plus non-selective (or cyclo-oxygenase-1) NSAIDs; proton pump inhibitors plus non-selective NSAIDs; misoprostol plus non-selective NSAIDs; COX-2 selective NSAIDs; or COX-2 specific NSAIDs--in reducing serious gastrointestinal compli...
متن کاملPrevention of NSAID-related upper gastrointestinal toxicity: a meta-analysis of traditional NSAIDs with gastroprotection and COX-2 inhibitors
BACKGROUND Traditional NSAIDs (tNSAIDs) and COX-2 inhibitors (COX-2s) are important agents for the treatment of a variety or arthritic conditions. The purpose of this study was to systematically review the effectiveness of misoprostol, H2-receptor antagonists (H2RAs), and proton pump inhibitors (PPIs) for the prevention of tNSAID related upper gastrointestinal (GI) toxicity, and to review the u...
متن کاملEfficacy and tolerability of naproxen/esomeprazole magnesium tablets compared with non-specific NSAIDs and COX-2 inhibitors: a systematic review and network analyses
Non-steroidal anti-inflammatory drugs (NSAIDs), such as non-selective NSAIDs (nsNSAIDs) or selective cyclooxygenase-2 (COX-2) inhibitors, are commonly prescribed for arthritic pain relief in patients with osteoarthritis (OA), rheumatoid arthritis (RA), or ankylosing spondylitis (AS). Treatment guidelines for chronic NSAID therapy include the consideration for gastroprotection for those at risk ...
متن کاملPractical approaches to minimizing gastrointestinal and cardiovascular safety concerns with COX-2 inhibitors and NSAIDs
Nonsteroidal anti-inflammatory drugs (NSAIDs) are highly effective in treating the pain and inflammation associated with osteoarthritis and rheumatoid arthritis, but it is well recognized that these agents are associated with substantial gastrointestinal toxicity. Treatment guidelines suggest that patients with one or more risk factors for NSAID associated ulcers should be prescribed preventive...
متن کاملDo cyclooxygenase-2 inhibitors provide benefits similar to those of traditional nonsteroidal anti-inflammatory drugs, with less gastrointestinal toxicity?
Two forms of cyclooxygenase, cyclooxygenase-1 (COX-1) and cyclooxygenase 2 (COX-2), act as rate-limiting enzymes in prostaglandin and thromboxane synthesis. Discovery of these compounds led to the development of drugs that selectively or specifically inhibit the COX-2 isoform. Although the COX-1 isoform is expressed at fairly constant levels in cells, including the gastrointestinal mucosa and p...
